??? Required immunizations? USHIP

Hmm... so i'm really curious since it seems no one has mentioned anything about this, and some people i know who are also going to uci haven't done anything about this either. I'm just wondering how many of you filled out this form? http://www.shs.uci.edu/forms/Student_Packet_2008-2009.pdf requiring immunizations? Is it required as it says? When do we need to get this in by?

Thanks for any replies

yeah you should do it.

do it before school starts.

but do you know what the exact deadline to get it in by would be? would i be safe to have it in by the 25 when school starts? or do i need it in sooner?

just mail it to them earlier.

i don't really remember.

i did it. never heard anything back about it (is there any way to check?) so i'd assume it's okay. i think i read that you have to have it done by the first day of instruction

is it only for students who wish to waive out of school insurance??
also if not can we do it at school??

nvm.................

The immunization record is required by California law, and in theory you could have a hold put on your registration.. The reality is the Student Health service gets most of the forms at the last minute, so you'll have a grace period. Just mail or bring the form to Student Health (behind Middle Earth). If you have an official immunization record and attach a photocopy, you won't need the signature of a licensed medical professional.

Unless you're an intercollegiate athlete or dance major, you aren't required to have a physical exam.

There are two good reasons to have a record at Student Health.
1. For your safety -- for instance, if you're in an accident or exposed to meningitis.
2. For the safety of the community

You could have it done at Student Health, but there's a charge for it and you might have to repeat some shots.

this is only required if you waived out of the schools insurance right? just making sure, haha.

i thought it was required for everybody

yeah, it's required for everyone.
