I'm typing up my MSU (Michigan State) application and this is what I decided to put in the "additional information" secton:

At the beginning of my sophomore year, I moved to [new high school name], which was the second time I switched high schools within a 6 month period. This change was a little overwhelming at first, and brought with it a small amount of anxiety that led to a sleeping disorder known as chronic insomnia. Because of this chronic insomnia, sleep deprivation was an issue during most of my 10th grade school year, as I was functioning on as little as 3 hours, and rarely getting as much as 6 hours of sleep per night during the week. My condition improved by junior year and the year started out great, but sleeping at night became very difficult after receiving the news that my dad was diagnosed with a rare lung cancer. My grades were on the decline once again, and the worst part of it all was taking my first ACT test on the day my dad was having surgery to remove the mass on his lung. As my dad's condition improved, so did mine. Soon he was cancer free with just a few chemo treatments ahead of him just to be sure. I began sleeping better at night as the school year wound down, and I even dramatically improved my ACT score the second time I took it. I am currently receiving treatment for this condition that seems to come and go with anxiety, and I continue to get better everyday. My current prognosis is that I will be sleeping on a normal schedule again by October 2012, and I'll be doing everything I can to make sure it stays that way.

Is this the kind of thing that goes there? If not, I'll change it. This is just an unedited draft that I typed up in 5 minutes, so tear it apart. I'd rather you hurt my feelings than submit something that sounds ridiculous.
