Is it ok is I just talk about one thing, my interest in cancer research, throughout the entire essay? Or do I have to include extracurriculars and activities also?

You don’t have to include your activities if you don’t want to.

What is the word limit on the essay? If it’s short (100-300 words) I would say it’s okay to just talk about one thing. However, if it’s on the longer side (300-600 words), it might be better to talk about some other things. Do any of your extracurricular activities show an interest in research? Maybe you volunteered at a hospital? If you can weave your experiences of these activities into your essay, it will make it a lot stronger.
