List of things to do prior to arrival

<p>I just wanted to start this both to inform, remind and so that if some of us didn’t receive something we can get it in:</p>

<li> Service Member’s Life Insurance (send to Academy)</li>
<li> Service Member’s Death Benefits (send to Academy)</li>
</ol>

<p>These two I have the names off a bit, so someone correct me. They’re essentially the same thing, filling in names of people to receive benefits. The one is term life insurance, the other for pay.</p>

<li> Tailor measurements (send to Academy)</li>
<li> Background check form (rough draft, just a gathering point, bring to I-day)</li>
<li> Final college and/or high school transcript (send to Academy) (college by June 10, high school don’t know what the date is)</li>
<li> Set of bitewing radiographs and a panoral radiograph (bring to I-day)</li>
</ol>

<p>I’m a bit confused about 6, because at one point in “Instructions to Appointees” P.O. box version, they act like it’s just for those with orthodontics, while in another spot they act like everyone needs it.</p>

<li> Proof of Citizenship (to accompany Form 86, which is Bullet 4) (Bring on I-day)</li>
<li> Set up checking account</li>
<li> Take language entrance exam online</li>
<li> Practice math exam online</li>
<li> Send college transcripts to english and math departments (if applicable)</li>
<li> Set up academy checking account</li>
<li> Buy and break-in boots (bring on I-day)</li>
<li> Molded cleats for intramural sports (bring on I-day)</li>
<li> Sign up for bed and breakfast (if applicable) through AOG</li>
<li> Eyeglass prescription to I-day, black lanyard to I-day, and prescription form received and returned (if applicable)</li>
<li> Call Academy Travel Management Office to request airline ticket</li>
<li> Call about sponsor family if not contacted to arrange for that</li>
<li> General Questionaire</li>
</ol>

<p>General questionaire, death benefactor and language test can be found in the same place (will need to check on the death benefactor being both online and in paper form).</p>

<p>If you add to it, copy what I had (minus my remarks outside of the bullets) and add to the end or modify it.</p>

<p>Check out the "Data Collection" thread. It has a link to a general questionare, points of contact (in case of death/serious injury) form and the language placement test.</p>

<p>Once you start one you have to finish it in one shot or redo it completely.</p>

<p>Regarding number 6 (Dental X-Rays), I'm pretty sure that everybody needs them. Quoting the Memorandum from the Dental Squadron: </p>

<p>VERY IMPORTANT: To expedite completion of your dental evaluation, I request that you bring to the dental examination one set of current (taken within 12 months) bitewing x-rays and current panographic x-ray. Copies of digital x-rays printed on paper are typically of poor diagnostic quality, we strongly prefer conventional x-ray films.</p>
