<p>Thanks to those of you who replied to me in the other LOA thread but I don't want to threadjack so I started a new one. In the letter it says I need to send the Candidate Personal Data Record, the Rquest for econdary School Transcript, the School Offivial's Evaluations, the Candidate Activities Record, and the Candidate Statement by 10/15.</p>

<p>As of yet I have never received any of these forms. When or where can I get these?</p>

<p>My daughter called someone yesterday to ask those very questions. She has received an LOA. She was told that those forms would be coming soon and if she didn't get them within 2 weeks to call back. She spoke with the admissions counselor for the area in which we live. That information should be somewhere on your letter.</p>
