<p>I would use the common app's version. It's pretty basic. Just a line for the course name, prof name, date, grade, and comments all on the same line.</p>

<p>HELL NO! It would have been the best thing that happened to me if they did.For a moment, i felt like creating my own forms.I've dropped Colgate from my list cos of this. I thin we need to inform commonapp about this, to save future applicants</p>
