I am going to encourage my daughter to sign some HIPAA forms, so that in the case of a medical emergency, the doctors can share information with me. It looks like HIPAA forms are specific to each facility, so I’m trying to figure out which facilities we should plan for. (And then hope we never need this!)
You might have her sign a medical power of attorney, that would allow you to talk to the doctors and make decisions as needed, regardless of which medical facility she is in. She can also sign a general HIPAA release form that medical facilities should honor, even if it isn’t their form.
When Goddard is ill-equipped to handle medical issues they typically refer students to [Norman Regional Hospital](Norman Regional Health System). The address of the one nearest to OU is 901 N Porter Ave, Norman, OK.
Likewise, every adult 18 years and older should have a Health Care Proxy. You can fax a copy to the health office at her school.
Speaking from personal experience: DS ended up being transported by ambulance in April from Traditions East to Norman Regional Hospital.
The ER doctor called and spoke to me personally, which was much appreciated as I only had his friends to rely on for information.

I know my son was conscious when she called me, but I do not know if he gave verbal consent. He had accidentally overdosed on his ADHD meds and can’t remember exactly what was said at the time.
Of note: my insurance covered the ER doctor charge and ER hospital charges at an in-network rate. The ambulance was NOT covered at an in-network rate. However, I sent my “Explanation of Benefits” to the company and asked that they forgive the difference between the “eligible charge” and the actual charge. Thankfully they did agree to waive the difference.

I was trying to understand the 3 items you mentioned and could not get a clear understanding of what is the difference and where to obtain the forms. Google search is coming up with so many variations of forms and not able to decide is Proxy and Directive is one and the same.

Proxy form has so much variation. The form in OU Medicine site is simple and just a page. Not sure if that form is sufficient or need to have all the 3 forms.

ok gov site form says directive but with in that it has living will also. Also it expects to sign these doc in OK county office.
