I just got my NMF confirmation [sigh of relief] in the mail today, and I'm a bit confused on the subject of college-sponsored scholarships.

Say I put down College XYZ as my #1 college (and they are a sponsor). In April, I am awarded $1000 per year to College XYZ. However, for financial reasons, I decide I cannot attend College XYZ and must instead go to State U. Does this mean that I get no money from National Merit at all?

But if it is mid-April and I change my mind based on the aid package the college gave me, is there still time to change it? Would I still be considered for the main $2500 scholarship if I changed my mind at that time?

You need to stay undecided until you are sure, cause once you are offered an award from the first choice college, you have to go there, or forfeit the money. National Merit decides if you get the 2500 one time award and it can go anywhere you go.
