If my career interest is to become a pediatrician, what category is it under future plans-- career interest in the Common App?
Is it a physician? and is the highest degree needed medical?

Yes it’s a physician. IIRC, there’s no MD option, just doctoral level (which MD would fall under).
Yes, pediatrics is a specialty within the auspices of being a physician. Like Radiology, Psychiatry, Anesthesiology, dermatology, geriatrics, internal medicine, ob/gyn, etc…
