<p>I knew there was one but don't have one anymore that came with all of the mailing stuff. I just assumed that it wasn't really needed since on the common app there is no supplement or anything that seems to be like the form 1 or what ever it is called.

<p>If you go to the Common App online, you can download the Pomona supplements, print them, and send them in. My son just sent in the pre-app and the signature page and plans to submit the rest of the supplement online.</p>
<p>1) Yes, you need to fill out and send in the form. Pomona has several supplements to the Common App and you need to have them all turned in before your application is complete.
2) You can download the form from the Pomona admissions website, of do them on-line at the Common App website.</p>
<p>Actually, the Preliminary Application isn't required. If you've already submitted your Common Application, then there is no need for the Preliminary App. All the supplemental forms that are required have "required" in the title, such as the "Required Signature(s) and Optional Photograph" form.</p>
