<p>When I previewed my application, the 3A form regarding the science supplement (nor any of the other supplements such as PLME) did not show up. I submitted my application online. Does this mean that my form 3A Science Supplement was also submitted?</p>
<p>Might this be because I chose Mathematics as my major in the application? However, I stated I wanted to study math in conjunction with physics, so I wrote the statement anyway. I just sent them an email containing my science statement to be safe.</p>
<p>I think that the instructions are to only fill out Form 3A if your primary interest is science.</p>
<p>It says: "This form is required if your primary interest is a Sc.B. or A.B. degree in Chemistry, Computer Science, Engineering or Physics." It sounds like you chose math. Then again, the way it is phrased makes it seem as though it's optional for non-science majors.</p>

<p>I still wasn't very convinced (because it does say that the PDF preview is exactly what the admissions board is going to see!), but I just submitted it (ED).</p>
