<p>I know this may sound stupid but anyway. How is the layout for the teacher evaluation meant to be?. Is it written in the form of a letter? for example 'to whom it may concern .......' or can my name be put at the top and the name of the teacher and then the evaluation. I tried to look for a sample but couldn't find any for my teacher.</p>

<p>If it’s a recommendation for inclusion with the Common App, there’s a specific form for the teacher to fill out. Use the “school forms” section of commonapp.org to “invite” your teacher to make a recommendation. If you include the teacher’s email address, the teacher will get an email with a link to fill out the recommendation form online. If you don’t include the teacher’s email address, the teacher will be an “offline recommendation” and there will be a form for you to download, print, and give to the teacher to fill out and mail to the school you’re applying to.</p>

<p>If you’re talking about an actual letter that is attached to the main Teacher Evaluation form, teachers should know how to write those. You don’t have to worry about it.</p>
