For those applying to more than one school, as most of you probably are, do you use the same recommendations for all the schools or do you ask other/same teachers to write new ones?

Usually you use the same ones. If you are applying to different programs at different schools, you might ask your teachers to write two slightly different letters to appeal to the differences-- maybe mentioning different clubs, etc. But ordinarily, letters should be good and general enough to go anywhere.

A recommendation is not just the letter. There is also a form where the teacher rates you on different scales.

Is that form particular to the school?

See -- I don't have many teachers I want to write me a rec. I have three in mind, maximum. And one had me for an elective class (Journalism) and is my newspaper advisor -- so she probably will not be the person I ask because usually they want a core teacher. So if I apply to 6 schools - I should ask these two teachers to fill out 6 forms each? Is this normal? I just don't want to ask them for too much, yanno. But I know these two teachers will write me excellent recs....
