So a long time ago back in the days our first SAT, College Board requires us to fill in a bunch of information about ourselves. Well back then I didn't know my unweighed GPA so I selected 4. I just got my latest score report and noticed that I never changed it. Is this a problem? On my score report it says my GPA is 4.0 but in actuality its around 3.5. What should I do? Can I send these scores?

Colleges don’t even look at that information. When you go to apply to colleges, you have to send your transcripts, which are reports of you’re entire high school career: Every class you’ve taken, the (semester) grades you got in the class, and your current weighted/unweighted GPA and class rank (if applicable [some schools don’t report rank]) up to the end of your junior year.
