SHIP/Health Fee Waiver

<p>Has anybody used Medi-Cal to waive the UC SHIP/Health fee? I have Medi-Cal with a share of cost of $143. Does it qualify for the waiver? Where can I get the info necessary to complete the online form? I did call my local Medi-Cal office and email UCSD student health office, but they can't answer any of my questions. </p>

<p>And I am from the Bay Area. What should I do with my Medi-Cal plan when I move to San Diego? Does it cover medical service in SD?</p>

<p>call your provider and ask them all those questions. I dont have medi-cal but I had the same questions and my insurance company was incredibly helpful in answering all my questions for me :)</p>

<p>^^i agree. my waiver was approved. i didn’t know what the criteria was so i just did the application. they just ask you “does your healthcare provider offer you this, this and this?” and if the answers are yes then you will be approved.</p>

<p>I believe you just need to check yes to all the “Is ____ covered?” questions and make sure your ranges meet their requirements. I think all those maximums (annual, per injury, and lifetime) need to exceed 400K and the out of pocket aka deductible has to be less than $7000 or so. HSA is optional, I think. Policy # should be on your insurance card. I believe that’s it.</p>

<p>Hey! I’m with Medi-Cal/Health-Net as well and I sent in my fee waiver last week and got it approved the very next day. The way I filled it out was that when it asked for address and such, I just searched up the closest Medi-Cal/Health-Net provider/office to La Jolla, filled that info in for address/phone number etc. And for other questions pertaining to the cover ranges or whatever I just put the last option which should be something like my plan does not have a blahblahblah or whatever. </p>
