<p>I think you should let the teachers worry about that. But a generic/clean letter with simple headings with your name somewhere and their name is good.</p>
<p>Supplement letters should be on some form of letterhead...many schools will not accept the letter if it isn't. The letter should also include your applicant ID or social security number.</p>
