<p>No special form. I'd ask the person writing the rec to write "Supplemental Recommendation for Student X" across the top of the letter.</p>
<p>I just printed out the Common App Teacher Eval, and wrote "supplementary" right on the top of it.</p>
<p>Yale 'strongly discourages' additional letter of recommendation, but does say that if you feel the need 'go for it'.</p>
<p>(No....that last one isn't really a quote)</p>
<p>But they do ask you label supplementary materials as such.</p>
<p>Its a pain that Yale doesnt confirm on the app status page that <em>check</em> they've received the supplementary junk that you've sent.</p>
