<p>I'm a bit confused. Does the teacher evaluation form on the common app count as a teacher recommendation? And if it does, does the teacher need to do anything besides fill that out? Or do they have to supply an additional letter as well?</p>
<p>Also, does the teacher have to fill out 10 different common app forms for each college I'm applying to? Or can they fill out one, and then photocopy the rest?</p>
