<p>Some schools have a rather complicated process for faxing in materials. UChicago simply says, fax it into their number. Should I include a cover sheet or something with these documents? They say they want to recieve tax returns by fax, so I have a few questions.</p>
<p>1) Will the tax return be kept safe? What if I fax it in at a time when no one is there--could it be lost or misplaced? </p>
<p>2) What sort of identifying information should I place onto the tax return pages? I have heard everything ranging from DOB to SSN. Someone told me that DOB and full name is fine. What did you do for UChicago?</p>
<p>3) Should we include a cover sheet? Is there one? I could not find one on the UChicago website for a fax cover sheet.</p>
