student id on tax returns?

Hey guys?

On the Uchicago website, it said all tax returns need to be signed and have the student ID on the top. What if I don't know what my student ID is? Should I put my name on top as well? Also, does it mean that every single page has to be signed or just the front page? Does UChicago want a cover sheet or something?

Thanks!

Are you a prospective applicant? If so, use this page: https://collegeadmissions.uchicago.edu/costs/apply/instructions.shtml

“Please write the student’s first and last name, address, and date of birth on the top of the first page of any document you would like to send”

Only returning students must include their student ID (first year applicants don’t have one). Make sure you are using the correct requirements on the page linked above too – they are different for applicants and returning students. The deadline for some forms is Feb 1 (already passed), but it is not too strict, so hurry up if you haven’t sent any of those required forms yet.
