UCLA Change Report Help

<p>So I've just been admitted to UCLA and didn't know that it was necessary for me to report two of my NON UC-transferable classes on the app. I got a P in one of the classes, and a W in the other one. I didn't report the W because I already retook it and replaced it with another P.</p>

<p>I called transfer advising and they told me to fill out the Provisional Problem Form, however, there is nowhere for me to report my specific scenario because the classes I took were taken last year, and there is nowhere for me to report changes that have occurred from back then. Does anyone where or how I should report this to them?</p>

<p>Yeah, I did, but I’m not sure where I should fill it out because it allows me to make changes for Fall 09, Winter 10, Spring 10, and Summer 10, but there’s no where for me to report changes that have been made prior to that… :&lt;/p>

<p>I was in a similar situation and emailed them. Here was their response: </p>

<p>"Please fill out a Provisional Problem Form if you have not done so at the following link. For coursework taken prior to Fall 2009, you will need to use the comment boxes to report the changes.</p>

<p>An evaluator will review your changes and let you know whether the changes will affect your admissions. In the comment section you can explain the reasons for your changes. Please allow 10 working days for an evaluator to get in contact with you.</p>
