<p>I am trying to find the printable School report form to give to my guidance counselor. None of the guidance counselors at my school know where to find it or how to get it. I found one link for it but when i clicked on it it said "error". I need this done by November first, meaning it needs to be mailed to the schools very soon. Thank you so much for the help.</p>
<p>The only way to access it is to invite your counselor electronically through the common app and have them opt out of electronic submission. Once he/she does that, your common app account will update to give you access to the pdf forms customized with your name, school code and common app ID.</p>
