okay, so my dad scanned all 13 pages of the questionnaire and turned it into a word document that one can edit. this way, one can type in the answers and just print it out. a lot easier then handwriting or answering on a different sheet of paper, wouldn’t you say?

here is the link (i uploaded it onto google docs). all you have to do when you get to it is go to “file” and then “download”, where you can choose PDF, Word, and others. once you have downloaded it, it is yours to edit!

okay, so i found out that you can’t download the file unless you are the publisher…me! well, that isn’t going to help anybody so i created a secondary google docs account and uploaded the file there. just go to docs.google.com

and then type in usafaform@gmail.com as the e mail address and usafacademy as the password.

once you are in just go to owned by me tab on the left, and you will see the document. from there you can open it up and download it.

it’s a bit complicated, i know, but it’s the only way i know how to upload something on the net and have everyone be able to download it.

some people are having problems with the questionnaire i put up on google docs, so i uploaded another one- this time it is compatible with windows word 97/2003 versions. the first one was only with windows 2007, or so it seems. hope this helps.

p.s. maybe a moderator or something could upload an attachment directly onto this forum?

oh yea, the new version is called “questionnaire- word 97/2003 version”
