PROFILE (If under 30 and applying for Full Funding, or over 30 and applying for “HHS” funding.)
UCSF Application for First time applicants*
Social Security and Citizenship Information* (If required by FAFSA)
Student’s signed 2006 Federal Tax forms (including all schedules and attachments)<br>
If student is under 30, and applying for Full Funding, submit both parents’ signed 2006 Federal Tax forms (including all schedules and attachments)
Other documentation may be required by your financial aid advisor. Please keep your mailing address and email address current with the Office of Admission & Registration (OAR) to receive communication from our office.
Federal Stafford Master Promissory Note (MPN), with choice of lender. (Not required to apply for financial aid, but the Stafford Loan is a standard part of UCSF Financial Aid package and it is recommended to submit the MPN after submitting your application.)

<p>Not wanting to get redundant, but most students will NOT be considered independent by the medical schools mentioned and parents’ income IS considered and is certainly NOT irrelevant. USF requires, Fafsa, Profile, and BOTH parents’ tax returns.</p>
