What Physical Chemistry Grad programs to apply to?

Ok, well here goes. I need some advice on what universities I should look into for graduate studies.

Me:
Graduated from a top UC school with a 2.8-2.9 gpa (I know it's low)

gre: 164 verbal, 165 math. havent taken subject test yet
I've always taken more classes than I needed to in order to push myself.
I've done several years of undergraduate research, presented my research at several conferences, and am pending a first author publication.
Classical piano training for >20 years
I've been involved in several officer positions in campus organizations
My coursework includes graduate level p chem courses which I got As in, and a few upper division physics courses which I did well in.
I am super passionate about p chem and qm.
I've got a professor who is willing to go to bat for me, and make some phone calls around to help me get in.

Thoughts?
