<p>Does anyone know if the first page of the teacher evaluation form is supposed to have a big blank space at the end? Can anyone check?</p>
<p>How big is "big"? I got a paper copy in the mail and it had about two inches of blank space below the teacher contact information. There shouldn't be anything printed below the line for teacher's phone number.</p>

<p>Also, does my name and Social Security # have to be written on the top on a letter of recommendation if it is being mailed together with the common app Teacher Evaluation form?</p>

<p>I have heard that they do not in general. Particularly if they have a lot of reco's to do. With kids doing around 10 apps each, the load gets very high for teachers. Some econmizing is critical or that is all they would be doing.</p>
<p>Is it strange for a college to require both a teacher evaluation form and a separate letter of recommendation? I called American University today, and the lady said that both the Secondary School Report and the teacher evaluation form had to have separate letters of recommendations with them. I called twice to make sure, and that's what she said.</p>
