advice on letters of recommendations. help!!

This might be a really dumb question, but I've never gotten rec letter from anyone before, so I'm not sure about etiquettes. I'm a high school junior.

When you are asking for a letter of rec to a teacher, what should you give to them? Just the recommendation form? my resume(summary of my achievements?)? should i give them my transcripts, or a summary of things that I would prefer them to write about?
what did you guys do??

If I give them an envelope with receiving adress and stamp on it, what should I write for the sending address? my name or teachers' names??

THanks, I would really appreciate any help~

bump bump!

Give them the form and a resume. Don't write the sending address on the envelope. Your teacher can do that himself/herself.

Thanks!
Should i show them other parts of my applications (i.e. short answers?), I'm not really done with them yet.

I would write the teacher's name along with the return address on the envelope; if they're taking the time to write a rec for you, it's courtesy to make the process as easy for them as possible.

When I ask teachers for recs, I usually include the recommendation form, a self-addressed stamped envelope, my resume, a copy of my transcript, and a bullet-pointed list of my achievements in their class as reminders, and a list of deadlines. I also check with them individually for any additional material they might need.

I gave them my resume, an envelope with the sending address, and info about the program. Then, I gave a thank you note around 2 weeks later

jbc42, you give them all that stuff for summer program letters? that seems a little overwhelming unless you're applying to the big-time stuff (RSI, etc.)
