Application Fee Waiver Help?

I'm sorry, I'm just so unaware of this college application process, first generation and all.

On the supplement, it says:

If you indicated on your Common Application that you would be applying for an application fee waiver, please provide your combined family income in U.S. dollars, the number of people living in your home as declared for tax purposes, and any other pertinent information.

How would I do this?

Would I type it on a word document, like "My combined family income is...."?

Or would I scan official documents?

And what do they mean by other pertinent information?

Any help would be appreciated. It's the last day and I'm stressing out like mad.

your parents IT RETURNS for 2008 or 2009 will have these printied on them, if both your parents work, you might wana add the sum up and write it on a sheet of paper, alongwith ur name and no of dependents and any other information you wish to give them that helps your waiver.

i think it would be better if that sheet of papaer had information in points rather than it being a flowy wordy letter. straight and to the point imo

hope it helps x
