<p>Teacher Evaluation Forms- What is this for? Do you need to hand these forms to teachers or is the LOR sufficient? If a kiddo applies to 12 schools, I am sure a teacher does not want to complete a teacher evaluation form for all 12 schools (or even once) after they have already written a LOR. When would they have time to teach?</p>
<p>You give the form to the teacher, along with an addressed envelope and postage.</p>
<p>The teacher will take it from there.</p>
<p>I think the convention is that the teacher simply fills out the top of the form and then attaches a copy of the letter of recommendation, but I am not sure because my kids both waived the right to see their recommendations.</p>
<p>Ask your school. Ours doesn’t use them; they have one form that all the teachers use and goes to the various colleges. (I was glad about this since S’s handwriting is messy and this way they won’t have it on the forms!)</p>

<p>Ask your school. Ours has the teachers turn the forms in to Guidance, who then makes copies or sends them out. I think most teachers use the Common App form - no one wants to ask a busy teacher to re-copy the letter of rec into a whole bunch of school-specific forms. Although D is having her GC and one of her teachers do one for her ED school, and using the Common App form for the rest of her schools. Her ED school doesn’t take the Common App.</p>
