<p>Ok, I had seen this form but I had just seen the first page and overlooked the fact that this particular PDF had 4 pages and not 2. </p>

<p>I did, however, send them the Intl Supplement to the School Report. Which asks all the questions this one does. </p>

<p>Don't really know, I was a little confused about this as well; I ended up asking my GC to fill it up and delayed sending my Harvard forms by a day. The predicted marks bit is the same, but the comparison to other students isn't. I'd say send it with your Mid year report.</p>
