Art Supplement? How does it work?

I'm really confused about how to go about putting together an art portfolio. Most of the colleges I'm applying to either don't require one or don't explain how to put one together.
Should I make a slideshow of all of my art? Keep them in separate files and put them on a CD? Print them out and mail them? I don't think I can mail them because I wanted to include animation as well..
If I put them on a CD, what do I title all of the files?

Also, I think I might want to include a music supplement as well. Do I do that separately? Is there any way I can combine the music and art supplement? (ex. have a slideshow of my artwork with me playing clarinet in the background)

Sorry there are so many questions! I just don't want to mess up when I'm putting this together.

Only send a supplement if you are really talented in that area because they have to go through many of them. If you decided on sending a music and art supplement send them separately because they will most likely give your music supplement to the music department and art supplement to the art department for review. For the art supplement it may be best to send it on a CD like a slideshow and as long as the CD is readable it really doesnt matter the type of file.

The schools that require art portfiolios generally have instructions on their websites on how to submit–why don’t you check some of those out?

Some of my artsy friends made an online portfolio and is going to include the link in the application. I thought that was pretty clever…
