Hello, I have a couple short questions about this recommendation

1) I plan on printing out the form and letter and sending it with a CD of my visual arts portfolio (all in one envelope). does the recommendation letter have to be sent in with everything, or can my instructor send it in seperately, (maybe even a little after the deadline?) If it is sent in separately, should it be said somewhere it is part of my arts supplement?

2) My instructor is someone who I have taken classes with at a local art museum since my sophomore year in high school. He is not an instructor at my school. Is this alright?

I can't really accurately answer number one, because I'm doing mine online.

As for number 2, yes it is absolutely OK :p
In fact, I think most people don't have their music/arts/whatever instructor from school. I'm using my composition teacher who lives a half hour away (also began meeting with him my Sophmore year)

As to #1, the recommendation letter could be sent separately. In fact, if you waive your rights to see the letter, it probably should be sent separately.

a question that might seem silly.. if this is not a teacher from my school, does he have to fill out an official form from common app? or any form?
