This is what the supplement instructions say:

sculpture, architecture, photography, and other
mixed media. Send 20 examples of your artwork

may include details and various views of the same
objects. Encase slides in 9" x 11" plastic sleeves.
Indicate orientation of the image by writing “top”
on the slide mount. On the mounts or on a separate
page include your name and the work’s title,
date, medium, and size. Please number each
slide. Examples of time-based media (videos and
computer animations) should be submitted on CD
or DVD. The outside should be labeled with your
name, title of piece, medium, total running time,
and date of completion.Work received that does
not conform to the above specifications will not
be reviewed.

Theatre—Send a résumé, VHS tape, CD, or DVD
of a performance of two contrasting monologues (less
than five minutes total). Introduce the tape, CD, or

Looks like the answers are yes and yes.

brain, if your question is "can you send fewer than 20 slides?" then I'd say the answer is yes. Just send a reasonable number (say 12+) and make sure they represent your strongest and recent work covering a range of media. Drawing from life is especially valuable. You may also submit details of an individual work.

I would suggest putting together an accompanying arts resume which would include your slides, a personal statement, a list of your accomplishments/achievements, classes taken with grades and perhaps an additional recommendation from an arts instructor or mentor. The admissions committee will turn this over to the art and/or theater department for evaluation. Even if you don't intend to major in art studio or theater, these are valuable ECs.

I'm a freshman here and I don't want to mislead you, but when I submitted the arts supplement for theatre, I only had letters of recommendation and a resume. However, I believe it is recommended that you include a DVD/tape.
