<p>Your teacher will write a letter on a separate sheet of paper on your school's letterhead. However, he/she will also fill out the simple grids and information requested by each college's teacher recommendation form...so,</p>

<p>Check with your high school. They may have their own system of processing forms. My kids school took care of it. My kids HS wanted to process the entire packet. They discouraged on-line applications as too many kids applied on-line, didn't tell the HS and the school didn't forward transcripts-teacher Rec's etc. Our HS gets the teacher rec's and sends it in with the whole package including application, transcript-guidance counselor report etc. I don't think colleges want to receive teacher rec forms from the student as they could have been tampered with. Check with your HS, I am sure they have a method as to how they want you to process applications.
And yes- I am sure the same Teacher Recommendation is used for all the schools. It's probably xeroxed and sent along with the transcript. I do not think any college is going to expect teachers write up separate recommendations on the individual college form.</p>
<p>I don't remember how they work for undergrad, but for grad the teacher has to sign over the envelope seal for each rec, and therefor the teachers would have to do the duplicates and everything. They don't really want you to read the recs, so you shouldn't be handling them.</p>
