Cancelling my SIR?

<p>In order to cancel your SIR, send a letter to: </p>

<p>Please include your social security number, date of birth and your signature on your request for cancellation. We cannot cancel a Statement of Intent to Register via e-mail. The $100 deposit is not refundable. </p>

<p>It says I have to do that to cancel my SIR. What else should I add? Do I just give them a piece of paper with those information on it? :/</p>
