You should probably take the New SAT, as the old one will be phased out by February when you apply.
However you are match ( UCSD is a safety) for all except Stanford and Pomona. Pomona Low Reach, Stanford is a Reach.

Your profile has virtually no weaknesses at all and is all around very impressive in terms of both academics (which are top notch) as well as ECs (centralized around your intended major/pre-professional track). I would say you would have no trouble getting into most of these schools except for Stanford, which is a reach only because it is a reach for everyone. Overall, I would say your chances are VERY good for all of these schools. Keep up the great work!
