Chance Me?

I have been thinking about applying to a boarding school and I know I have to start applications this week.

I'm going to apply to the Winchendon school, St. Johnsbury, and I might apply to Loomis (I'm definitely not going to get in)
This year I've just moved from one of the worst and less challenging towns to one of the most challenging and rich towns (I will be moving again at the end of the year.

I never had to study or anything at my old school and passed with straight A's and now my grades are horrendous.

My grades: (school is block period)

First semester:
science: B
Math: B
Language Arts: A

Second semester (currently in) :
Science: B
Math: just brought an F to a B (my teacher will be assisting me more in everything, He also gives extra credit) I'm now extremely motivated to get a B and I'm pretty sure I can do it.
French: A
What would my chances be if this was an A,B, B, and A

I'm 50% Costa Rican and I am fluent in Spanish

Ec 's (lacking):
Girl Scouts- 5 and 1/2 years
Dance- 5 years (started when I was 5, I have a bad knee)
Saxophone- 1 year
Violin- 1 year
Community service- have over 100 hours and I started when I was in 4th grade
would be starting field hockey but this school does not offer it (doesn't have sports)
will be starting JV hockey if I got to one of these schools.

I already went for my tour and interview.
I'm very nervous and embarrassed to have my L.A. teacher and my math teacher fill out the recommendations because I feel as if they don't think I could make it.
I'm also going to take my SSAT in 1 week.
I don't know if this matters but I have been seeing a therapist for moderate/severe depression and anxiety and will be sent to a physiatrist soon and I will be evaluated for ADD also.

I've realized how challenging my school work has become and I'm study crazily now,
In all honesty how much of a chance do I have. Should I even try?
Thanks

I forgot to add that I am in Algebra and my teacher teaches it high school level and teaches in college fashion.

Good luck!
