This year, sophomore year, I was accused of cheating on a physics lab and it went on my school record. Would this completely kill my chances of getting into an ivy? Also, would it destroy my ability to apply for NHS? My grades are good, and I am involved in many clubs.

About the cheating, my physics teacher only grades the lab stapled to the top of the pile for each group, and not knowing one question, I hurriedly copied a number off a group member's lab.
It's my first offense?

Supposedly they look at the applications as a whole, so if you have some stellar stats, they **might consider. Don’t get me wrong, that looks really bad, and I would suggest appealing that mark. But technically, everyone has a chance.

This fabled “permanent record” will not be seen by colleges, or by NHS. Unless of course your counselor decides to mention it in a recommendation.

I had the same situation my sophmore year. I got rejected from NHS. Honestly dude…NHS really doesn’t do much for you. You can do other things that are more impressive than something that almost any high schooler can get into.

For colleges, you will need to explain this incident in the Additional Information section. Make sure to fully explain the situation and your remorse. Do not make excuses.

Ideally, your GC will mention this in her rec as well alongside an explaination that this event was an outlier.

were you suspended? put an any sort of probation? because you will have to answer this question on the common ap:

Have you ever been found responsible for a disciplinary violation at any educational institution you have attended from 9th grade (or the international equivalent)
forward, whether related to academic misconduct or behavioral misconduct, that resulted in your probation, suspension, removal, dismissal, or expulsion from
the institution? Yes No

Go talk to your conselor. Find out what will be going to the college. If colleges will see it, and if you have an otherwise spotless record (as opposed to just appearing to have not gotten caught a 2nd time), then ask the counselor to address the issue in the counselor letter.

You should also explain in the area you can discuss other issues.
