1) Should we get a Science/Math teacher and a Humanities teacher to do rec letters, or is it equally good to get a Science Teacher and a Math teacher to do the recs.

2) On the recommendations form where it asks for "student street address, student city etc." Do they mean my home address or school address?

3) Should we include first quarter marks with school report form (did prety badly), or is it equally valid to just submit final marks in first semister with mid-year report?

1) i think you should be conscious what are you interested in…if you want to do science, math, then by all means that is fine to get both from same area (don’t pick a diverse person just for the sake of it), if you have multifaceted interests certainly show them. But the only way they know you can be a physicist is if they know how smart you are in comparison to your peers from a math/sci teacher. Second, if you don’t have a good relationship with a humanities teacher, don’t try it. Pick folks that can speak well about you with real knowledge. Third, CU cares a lot about how you operate in class, how you participate, how you impact classroom dynamics. So you should pick writers that can speak to these aspects.
