<p>I know many people asks these questions repeatedly but I just am going to go ahead and ask b/c I don't have time to search through.</p>

<p>College Official's Report: I am a transfer student from a Community College, our advisors are never the same and they do not know us personally. I don't think a single advisor has been there for more than 2 years. They see us as #'s not as individuals, and they usually do everything in their power not to have to interact with us. So I don't understand why I have this form filled out as it wouldn't even make sense especially coming from a CC.</p>

<p>Instructor Evaluation: Easy not a problem, I know a few professor's who will have no problem filling one out. </p>

<p>School Report: It says it is for a secondary school counselor. I don't think a transfer has to fill this out, right? I left HS over 6 years ago, I don't think my former counselors are there anymore.</p>

<p>Mid-Term Report: Why? We don't have mid-term reports at our college. I finished my Fall term. They won't be able to see my current Spring term grades so far.</p>

<p>So basically the only one I need to send it the Instructor Evaluation, b/c the other ones really don't make sense for me to send.</p>

<p>Why don’t you check with the schools that you are applying to?
Different schools have different requirements.
Some don’t need School Report as far as I know.
For Mid-Term Report some schools say you can send it when grades are out or have your instructors evaluate your current performance in class at mid-term.</p>

<p>Yeah, after I posted I finally found all the info I needed on the school’s site. </p>
