I'm planing to apply to eight universities/Colleges. Do I need to provide 8 teacher evaluation forms and 8 teacher school reports for each teachers and counselor? Or do Commonapp send my teachers them by mail?

Check to see if your school does recommendations online through either the CommonApp or Naviance, this would eliminate the need for the paper ones all together.

If not, then provide one copy and your teacher or counselor can copy them as many times as needed.

Our teachers require that the student provides the envelopes already stamped and addressed and the generic information on the teacher rec form filled out so they can fill in the evaluation and copy it multiple times. They don’t even want to have to fill in their name, contact information, etc…Seems like a reasonable request given the number of recs they have to write.

By the way, they also require that the student submit an information sheet that answers questions like:
- What are my strengths and what weaknesses have I overcome?
- What are my academic and professional goals?
- What can I contribute to the schools that I’m applying to in terms of unique perspectives or experiences?

Thanks M’s Mom. I have a separate form about all of my extracurricular activities. This good enough?

David’s review :(APEX, inspires, create, achieve) – Apex is a four year program for high school students (Grade 9-12). APEX is basically an Asian-American program where it hosts colleges’ tours, college essay classes, mentoring programs, SAT prep classes, and more. As an APEX student, I’ve learned so much during the course of these four years, not only did I learned from the best of the best individuals, staffs from Ivy institutes, but I’ve also learned valuable intuitions/traits which will guide me through-out life. In addition, the staffs and the student bodies are eminent, and helpful. You’ll never feel left out; everyone is affable and they’ll actually take time to talk to if, if they notice you’re new. It’s like a second family.

• Founded in 1965, the Chinese-American Planning Council, Inc. (CPC) is one of the largest non-profit providers of educational, social, and community services for Asian-Americans in the United States. It now services over 8,000 people daily through some 50+ programs at over 25 locations citywide. It administers community services, child care, youth services, workforce development, senior services, home attendant services, housing, and cultural services. CPC’s mission is to improve the quality of life of Chinese-Americans in New York City by providing access to services, skills and resources toward the goal of economic self-sufficiency and integration into the American mainstream.
David’s review: CPC also provide vast opportunities for High School students, and I’ve definitely took the initiative to participate in college credential courses, college tours, and college applications outlooks. CPC provides great informational sections for High School senior. CPC also collaborate with varieties of other organization, such as SYPE. They actually aid me through the whole summer job process. So overall, CPC has been a great help.
• The Young Men’s Christian Association (commonly known as YMCA or simply the Y) is a worldwide organization of more than 45 million members from 125 national federations affiliated through the World Alliance of YMCAs. Founded on June 6, 1844 in London, England by Sir George Williams, the goal of the organization was putting Christian principles into practice, achieved by developing "a healthy spirit, mind, and body”.
David’s review: I have participated in this program all though out middle school (grade6-8). The first two years, I was a participant where I was guided on HW help after school. And during my last year in middle school, I was consider a mentor, and it’s was my turn to guild and help student younger than me on HW. Looking back, YMCA has helped me learn coordination and leadership skills.
2. What extracurricular activities/clubs have you been a part of?
• Chess Club – Won a silver metal
• Math Club
• After HW Club
• YWCA
3. Did you ever hold a job (Paid position)? State the job title, description of your tasks, company and dates.
- The job is affiliated with SYEP (Summer Youth Employment program), where students from across New York send in their application and the lottery will define if they get the job of not; I was accepted. I decided to participate in theatre. I was positioned as the manager, which I organize the plot, the lines, and the participants. During the last weekend, our group did an official shake sphere’s play “Summer Mid Night’s Dream” for the public. The experience is beneficial and enriching. For one thing, I don’t “umm” constantly during speech or casual dialogue.
- What did you do during your summer months? (List and explain by grade level 9-12)
• Travel
• Study math, English and writing daily
• Pre-college section - What are your academic interests? Do you have any career goals?
- I’m planning on economic of engineering. As far as I’ve know, right not my long term career goals are still undecided, since it’s possible for interests to alter as time progresses.
- What do you see as your strengths in my class? What specific moments did you shine? (Site at least one specific example).
- Well, you’re one of my best teachers at school and hopefully you consider me as one of your favorite’s student. I have learned so much during the course of these years, and the knowledge, that I’ve acquired, will always stay with me throughout life. Also I consider you as one of my friends, instead of a teacher. That’s why I’m confident you’ll write an extravagant recommendation for me.
- What do you see as your weaknesses? (Be honest and specific)
- I have these “Eye Floaters”. It’s basically something detected vitreous humors that everyone has, but to some it seems more apparent. I can see these “hair like lines” constantly, on both eye and they are extremely annoying. I consider this as one of my weakness because these floaters prevent me to finish my work with integrity.
- What obstacles have you had to overcome in your education and/or family life?
- Making the right friends
- How have you grown or changed since 9th grade?
- During freshmen year; I was just an adolescent kid, worm doesn’t care much about school. I don’t have many friends nor do I have a social life. But over the course of these years, I have made some fantastic friends, and I have met some helpful staff and teachers. Looking back, I notice I’ve improved so much. I remember when I was still a sophomore in high school, and my mathematic isn’t so great, so I decided to commit myself to ameliorating my math skills for the whole summer after sophomore year. And guess what? I have become one of the top mathematic individual in Murry Bergtraum High School. Thus, I do believe that self-commitment is crucial for triumph and success, and Murry Bergtraum has helped me built that confident/proactive trait. Overall, this is the change over the course of these years – a change which can help me prosper in a university institution, as well as in life.
- What do you want to get out of the University/College education?
• An unique education
• Getting to know myself as an individual
• Meet new friends and inmates
• Job
• Travel
• Vast opportunities
• Experiences - Why am I (the teacher) the best person to writing a recommendation for you?
- Out of all these teachers I have over these years. I chose you because, I grown to like/respect you as an individual rather than as an teacher, who just teaches and goes on with another class. So please, write this recommendation letter for me; I’ll be sure to thank you!

Is this good enough, or is it too long?

Ok so i am an International student who came here from a tiny Country called Nepal. I came here 3 and a half years ago. I first came to this amazing school which I will not name because I am pretty sure there will be tons of Guys who will know me and I don’t want that. So the First sem was cake got good GPA besides that I learned about drinking and smoking. I was like the king of streaking and had too much fun. So I was broke by the end of 2nd semester because I had already used up the money for my third sem in the drinking and throwing party stuff. So I was too embarrassed and moved to Dallas TX. Dallas was nothing like the college town I was in. Every International student worked In gas stations and they went to community college which was really hard. Any ways I flunked a lot of classes while trying to balance college and my 70 hour work week (10 hr 7days) but I realized where i was headed So i got back to doing good in school and all. But now I only have a aggregate GPA of 2.47. It might go up a tiny bit in this last semester. I want help finding a school in Texas that has low GPA requirements and is sorta cheaper than others I have a fixed budget of 30 grand. I dont expect any financial aid. I am think of going to Univeristy of Houston Downtown only because most of the other universities have a minimum of 2.5 requirement. Can i still apply saying My last sem in community college is still in progress?
