<p>What is confusing. You have to submit whichever one each school ask for. Are you expecting uniformity? There isn’t uniformity in the US. These are private institutions; they can ask for whatever they want.</p>
<p>As for the UChicago form… isn’t there a supplement link on the online Common Application?</p>
<p>The U Chicago form will be on the U Chicago website.</p>
