Counselor Letter

Hi,
I was looking at the rec letter forms and i don't see one for counselors like in the common app.
Am i not seeing it? or it's just not there?
Id reallly like to include mine so if there isn't would I have to send it as an extra letter?

also, I saw examples of how the rec LETTERS should like in the mit website
but when i open the evaluation form, there are questions.

If my teacher has already written a letter, can he send his letter instead? or attach it?

The counselor “letter” is the secondary school report. If you look closely it parallels the Common App’s Secondary school report, which makes sense because they both have the same name. Look even more closely and you will notice that on step C of the first page it says “Summary and Recommendation: Attach an appraisal, assessing
personal and academic qualities that may be indications of promise
as a college student.”
So basically, MIT wants an attached rec letter. This makes the “Secondary School report” even more like Common App’s “Secondary School report”, which makes sense because, again, they have the same name.

Finally if one has read the directions from the Rec Letter Forms, one would have also seen that teachers are free to not fill out the questions, but instead attach a rec letter of their own.
