Disciplinary Record and Grad School

While in undergrad I was placed on probation for academic dishonesty. This was the worst mistake I have ever made. I am infinitely ashamed of it, have learned from it, and am trying to move on.

I know I must report it to any graduate programs I apply to.
(It is not noted in my transcript, but I did fail the class.)

So:
-If a graduate application does not ask about disciplinary actions, how should I tell them of this incident and my probation?
-If an application only asks if I have been suspended/dismissed, should I tell them of my probation anyway?
-My disciplinary record is kept for a certain number of years. If I apply to schools in the future, there will be no record of the incident. I am not sure how I will explain my mistake, since there will not be any other records. Is it worth mentioning?

Any advice would be greatly appreciated.
I am so nervous and ashamed of this, and I truly need your help.

I have the EXACT same problem, except I just received an “F” in the class, and was not placed on academic probation. However, I was told by one of the administrators in my department that the reason why i received the “F” is confidential and no one has access to that information, not even other universities I would apply to. Sorry I can’t be of much help, I am at a loss of what to do or say on my applications as well. My friends have told me just to come clean and explain in my statement of purpose what happened, and that I learned from my mistake and that it would never happen again.
