Do NYU require secondary school report? I am really confused

<p>On the NYU website its says</p>

<p>Secondary School Report and High School Transcript: Request that a counselor or school official at your high school complete this form and send it along with your high school transcript to the NYU Office of Undergraduate Admissions at the same address. Your high school transcript (or General Education Diploma) is required regardless of when you graduated.</p>

<p>Well, it only says the "transcript" is required not the form, so I called the admission office just to make sure. Guess how surprise, I called 5 times and three of them said they don't but the other two said they do (I spoke with both student ambassador and admission counselor). I really don't want to submit it because it's a lot of work. I only went to my latest high school for a year(exchange student) and did not graduate, so they don't know me much. Also, the stupid common app system do not let us complete the form online, how can I have my signature on the form if I send them the electronic copy?</p>

<p>If you read my post carefully, you can see that I called 5 times just to make sure, and I got two different kind of answers. That was the reason why I was confused. I asked my college advisor to call them today and the final answer is no.</p>

<p>I applied for 5 schools (including NYU) that use commonapp, I never used that high school form. I have COMPLETED the whole application process of University of Chicago and Cornell already without any problems. I feel like NYU is very flexible on these kind of things too, I get my TOEFL and ACT score waived. it just seems like no school really cares about that form as long as you have the high school transcript. and it sounds like the NYU officers can’t even answer you accurately. I guess in the safest way, you can send both but I am only sending my official high school transcript.</p>
