Since the courses are listed on my high school transcript, do I still need to request transcripts from the community college? I am afraid it will hurt my chances because it was my last semester of high school and I was going through some health and family issues, thus my grades were not as stellar as I wanted them to be.
Most of the time, transfer credit to another college does not affect GPA at that college for its purposes.
However, if you apply to professional schools like medical or law schools, all college courses and grades will be considered, including for GPA recalculations.
This can also be the case if you took courses at college X while in high school, enrolled in college Y after high school graduation, and apply to transfer to college Z. College Z will want to evaluate your record from both college X and college Y, even though your GPA at college Y is not affected by grades you earned at college X.

It really depends on the specific college. It will be obvious they were taken while still in college. Some colleges don’t give transfer credit for classes taken before HS graduation, others do if they appear on college transcripts. Some only care about grades if you want transfer credit, others care about all grades, but since they’re on your HS transcript, they’re already included in any calculations.
The odds are good that you need to send transcripts of any course beyond high school level. So, yes, dual enrollment, Community College, the works.
