Does Penn have a separate form for artistic supplements?

<p>I was just wondering if Penn has a separate form for artisitic supplement. Or is it the common app artistic supplement form.</p>

<p>I don’t think there were any specific guidelines. I sent in the common app supplement for my ED application.</p>
