Chicago essay topic seem complicated and hard to write,,
Even though I might be able to write topic 2 essay, I'm not sure whether I really want to go through that. I'm now thinking about choosing prompt 5 which I'll modify my 217 of 300 autobiography penn supplement essay..

Do you think essay that describe my future would be creative enough to satisfy Chicago admission officers? Or do you think it will turn them off because they might notice that I used penn supplement essay?

Also, do I have to state what my prompt is??

You never know really. The adcom might recognize it as another essay, then again they might not. I don't think just a description of your future essay will be enough for Chicago though. They have a history of wanting bold, interesting, and avant-garde type essays, not just "Hi, I'm John and I want to be a doctor because I love helping people."

If that was the case they would accept about 3-4 students a year. Look at the "Post your essays" thread here. The ones from people who claim to have been accepted are certainly well written, but rarely bold or avant-garde. The word I would use to describe them is "interesting"

Your essay does not and probably should not echo the tone of the prompt. The idea is not to fight fire with fire (or however you want to put it), but to internalize this intentionally weird concept.

Do the Penn one if you adore your essay like none other and aren't satisfied with your essay for option 2 (I think you should try it first). Do write the prompt in a way that it fits your essay. Do not use Penn's prompt wording.
