GPA Form Help Please

<p>I sent all of my documents, but I forgot to send my GPA form. On top of this I can't seem to find it anywhere! Is there some website that I can download the form from. I'm really scared right now because I got an email that says I haven't turned in everything and that they have already started reviewing, so I feel like it may already be too late. Can somone please give me any information concerning this. Thank You.</p>

<p>why dont you call the number on the bottom of the letter? It should be the number to your ethnicity rep at GMS. If they can, ask them to fax a GPA form to you or the school so it’s faster than mail.</p>
