Grad School and Disabilities?

I am looking into pursuing a master's degree in communication, but I have some serious health issues that among other things, cause me to have chronic migraines, feel extremely fatigued, and pass out a lot. I continue to work on getting my health under control, but I refuse to stop attending school. I'm doing fairly well in undergrad, all things considered (3.5, extra curriculars, research, etc.) but that doesn't mean it's not kicking my butt, health-wise.

I am worried that grad school is going to be too much to handle, in terms of work load, stress, etc. Has anyone else dealt with health issues/disabilities and grad school? How much are professors/schools really going to work with my health limitations? Should I write about my struggles in personal statements?

Any advice or info would be greatly appreciated!
